To begin approving videos, we need the following for EACH site:

The primary e-mail address for the account setup
We prefer to setup your RocketTube account for you. However, if you already set one up please provide the username
Webmaster affiliate program signup URL (if not already provided)
Description of site in less than 350 total characters
ONE (or more) 900x120 banners
ONE (or more) 503 x 137 banners
